MEMBERSHIP APPLICATION

GLSKA activities are shared experiences based on cooperation and mutual support. The organizers of GLSKA activities act as focal points to bring together the ideas, energies, and resources of the group, but are not formal leaders. They are not paid, and can take no responsibility for the safety or well-being of participants. Therefore, you should take the time to research and understand the degree of physical risk involved in these activities, and be sure that you are prepared for it, to make sure that you have all the required equipment and that it works, and be prepared to take responsibility for your own safety and good spirits throughout the activity.

To join GLSKA, please fill out and submit this Membership Application and the accompanying Waiver Form. (Memberships run from January 1 to December 31 of each year.)

EARLY-BIRD DISCOUNT:
Memberships received prior to March 1 will receive a discount of $5.00 for each year. (The chart above reflects these discounts.)

Incomplete memberships will NOT be processed. Complete applications must have a signed, dated and witnessed waiver and be accompanied by a cheque for the correct amount, signed and dated. (For family memberships, please list on the Waiver the members of your family who will participating and indicate those who are under 18 years of age and will be under your care during GLSKA activities.)
